A systematic snapshot review of quality of life research in medicine and health sciences.

Background: Quality of life (QOL) has become established as a central concept and target for research and practice in the field of health and medicine. However, QOL is a complex concept that is interpreted and defined differently within and between disciplines, and so is also the case in health and medicine. Thus, there is a continuing philosophical debate about the meaning and content of QOL and how it should be measured.
Aim: The aim of the present study was to perform a review of QOL research in medicine and health, and describe country of origin, target groups, instruments, design and conceptual issues
Methods: A Systematic snapshot review were conducted with literature searches in the databases Scopus, covering Embase, MEDLINE, CINAHL and PsycINFO, to identify research studies on QOL and Health-related quality of life (HRQOL) published during one random week in November 2016. To evaluate the conceptual and methodological rigor, we used the 10 predefined criteria developed by Gill and Feinstein.
Results: The findings show that QOL research is truly international, involves a variety of target groups, uses different research designs and a whole host of QOL measures. When it comes to evaluation according to the criteria developed by Gill and Feinstein, most articles still have a low-quality score; only 13 % of the articles provided a definition of the concept of QOL, few studies (6%) distinguished QOL from HRQOL. Criteria that were fulfilled most frequently were; i) stating the domains of QOL to be measured, ii) giving a reason for choosing the instruments they used, and iii) aggregating the results from multiple items.
Conclusion. Knowledge about QOL is important for understanding the consequences of illness and treatment, and for medical decision-making. However, based on the current evaluation of the methodological and conceptual clarity of QOL research, we conclude that most QOL studies in health and medicine have conceptual and methodological challenges.

Bakgrunn
Assisted Living-prosjektet (2016-2020) er et tverrfaglig prosjekt, og har ansvarlig forskning og innovasjon (Responsible Research and Innovation – RRI) som et overordnet rammeverk. Prosjektet er støttet av Norges Forskningsråd.
Mål
Målet er å involvere brukere i utviklingen av nye teknologiske løsninger som kan støtte eldre med mild kognitiv svikt i å mestre hverdagen. Studien foregår sammen med potensielle brukere som definerer egne behov for teknologi, stiller krav til produktutviklingen og evaluerer produkter og systemer som de tester.
Metode
Metoder for rekruttering var invitasjonsbrev, personlig samtale, og at husverten i omsorgsboligen oppmuntret beboeren til å delta i prosjektet. Inklusjon av deltakere foregikk i fem trinn: (1) Forskerteamet inviterte seg til et regulært husmøte i omsorgsboligen og presenterte prosjektet. Beboerne som møtte ble invitert til å delta i individuelle intervju, først (2) en spørreundersøkelse og deretter til (3) kvalitative intervjuer. I tillegg ble beboerne invitert til to (4, 5) «dialog-kafeer» med gruppediskusjoner knyttet til utfordringer i hverdagslivet og diskusjoner om mulige teknologiske løsninger som ble presentert ved scenarier.
Resultat
Studien er i oppstartsfasen. Vi vil presentere erfaringer og ideer med å rekruttere og involvere deltakere i hele prosessen fra å undersøke behov, utfordringer i hverdagen og til utvikling av teknologiske løsninger. Samproduksjon av kunnskap om hvordan noen eldre anser behov for teknologi og hvordan de kan nyttiggjøre seg velferdsteknologi for opplevelse av livskvalitet og verdighet vektlegges. Brukermedvirkning kan skape engasjement og deltakelse og er et steg på veien til å sikre ansvarlig innovasjon i utvikling av teknologiske løsninger.

This thesis draws attention to social interactions in dementia care within municipal healthcare services. Family caregivers are entitled to receive assistance and support tailored to their needs, regardless of ethnicity or culture. A major political goal in municipal healthcare services is to maintain solid and equal relations between patients, relatives, and healthcare personnel, ensuring a long-term partnership. Hence, the aim of this thesis was to explore perceptions of social interactions between family caregivers from minority ethnic backgrounds who have relatives living with dementia and healthcare personnel employed in
dementia teams and home-based care.
A social constructivist perspective underpins this thesis, which employed a qualitative approach influenced by the theoretical thoughts of Pierre Bourdieu (1977, 1979, 1980, 1990a, 1990b, 1992). The thesis is composed of three articles. Between December 2019 and June 2021, empirical data were collected from seven geographical districts in Oslo and seven surrounding municipalities. The empirical data were generated through 28 qualitative semistructured interviews with healthcare personnel employed in dementia teams and home-based care and with family caregivers from minority ethnic backgrounds. A reflexive thematic analysis was used to systematise the empirical data (Braun & Clarke, 2019). To enhance our understanding of social interactions in municipal healthcare, Bourdieu's theory inspired and guided the analytical process. This theory assumes that society is organised into a hierarchy of social relations and has served as a framework for understanding how municipal healthcare practices contribute to the emergence of power relations between family caregivers from minority ethnic backgrounds and healthcare personnel in dementia teams and home-based care.
It is suggested in this thesis that municipal healthcare services require further exploration of their institutional contexts. The findings reveal a mismatch between the ideals of high-quality care and support for people with dementia and their families from minority ethnic backgrounds and the external institutional frameworks in which such services are delivered. Despite Norwegian authorities' emphasis on cultural competence among healthcare personnel in municipal healthcare services, our findings indicate that family caregivers from minority ethnic backgrounds were more concerned with fragmented and poor healthcare systems in general than with cultural insensitivity. Moreover, we reveal tensions between healthcare personnel's lack of time and resources and their obligation to meet family caregivers' needs and requirements, leading to some family caregiver behaviours being viewed as more valuable than others. Healthcare personnel appears to be facing significant problems when dealing with diverse or different behaviours. When interacting with family caregivers from minority ethnic backgrounds, the importance of resemblance over diversity appears to have been more significant. The perception of family caregivers as either helpful or timeconsuming was also evident in many of the family caregivers' accounts. As a result, perceptions of ‘insiders’ and ‘outsiders’ were viewed as standard rather than discriminatory.
Our findings suggest that the inequalities in municipal healthcare concerning family caregivers from minority ethnic backgrounds were due to multiple discourses interacting with each other, thereby generating power dynamics. Despite Norwegian authorities' commitment to develop dementia healthcare services that are suitable and inclusive for everyone, it is pertinent to ask if the surrounding ideologies of municipal healthcare are in harmony with these ideals. In summary, this thesis argues that normative assumptions about what constitutes a family caregiver must be re-examined from an ethical and moral perspective.
