  This article examines digitalization in municipal welfare services for children and young people from the perspectives of municipal directors, frontline managers, and frontline workers. This study proposes a nuanced understanding of emerging digital work and workflows across multiple institutional logics. The authors argue that digitalization transforms professional discretion and institutional logics within Norwegian municipal welfare services for children and young people. Using a collective case study of two municipalities, interviews were conducted with 35 participants, including several levels of managers and frontline workers. The findings demonstrate the impact of digitalization on discretion, professional practice and institutional logics. Digital tools streamline record-keeping and enhance oversight, but they risk oversimplifying complex cases, increasing administrative burdens and constraining frontline workers’ discretion. The findings highlight a critical tension between managerial expectations of standardization and frontline workers’ need for nuanced judgment. The three mechanisms, replacement, blending and segregation, explain how an emergent institutional order “digital logic” interacts with established professional, bureaucratic and community logics. Finally, this study deepens the understanding of the involvement of digital logic when discretion is applied during work processes.

  In Norway, aftercare for young people aged 18–25 can be provided by the Child Welfare Service, Social Services, or both. We ask how services are regulated, implemented at the municipal level, and whether young people co-produce the services they receive. The approaches of co-production, governance, institutional logic, and coordination lay the foundation for the analyses of data from a document study and interviews carried out with professionals and leaders in one municipality. The findings show that governance mechanisms and institutional logic are parallel and competing. Sectorization is prominent, and overlapping jurisdictions are not avoided in the dominant regime combining public administration and market logic. The market logic is strong, favouring cost-benefit considerations, making the responsibility for young people volatile. The family logic is present at the professional level in both services; nevertheless, it must be worked with to be fully aligned with the professional and the community logic, and it is not dominant. The conclusions are that it is an open question as to whether young people co-produce the services they receive, and that the revised Child Welfare Act can strengthen the alignment of family, professional, and community logic.

  Purpose: Fragmentation in health and social care services can result in poor access to services, lack of continuity and inadequate provision for needs. A focus on integration of services are thus suggested to prevent negative consequences of fragmentation for service recipients. There are, however, few studies that explore the competence needed for integration of services in municipal health and social care organizations. This study explores which types of competence stakeholders require and how collective competence can promote service integration. Methods: This is a single-case study, and the data consist of focus group interviews and individual interviews with service recipients, family caregivers, professionals and managers. The data were analysed both inductively and deductively. Results: The analysis resulted in four main themes: 1) Knowledge about individual life situations and organization and system, 2) investigation competence, 3) person-centred collaboration competence and 4) facilitating competence. The themes form the basis for a collective competence framework that can promote service integration. Conclusion: As service integration involves a high degree of interlinked activities between professionals and organizational units, a collective approach to the concept of competence is presumably applicable. When service integration competence is approached as a collective attribute of a network within and between organizational units, the organization can facilitate this competence by encouraging an active exchange of knowledge between professionals. We also argue that service integration competence increases connectivity and interdependency between professionals and organizational units, and includes service recipients and family caregivers as legitimate extra-professional parts of the collaborative network.

  Introduction: The paper discusses the implementation of a digital workspace to facilitate collaboration in health and social services for vulnerable children and adolescents in eight Norwegian municipalities. The purpose of the workspace is to enhance collaboration independent of space and time. Collaborating services are schools, kindergartens, school health services, educational services and child welfare services. Methods: The data analysed are from semi-structured interviews with project leaders in primary care, responses of primary care professionals to open questions in a survey, and results from two questions in three subsequent surveys. Results: Project leaders held great expectations of increased collaboration. Variations were found regarding how far the implementation of a new workspace precluded previous methods of collaboration and whether retaining a familiar workspace necessitated strengthening resources to negotiate using the workspace. Organisational and professional cultures hindered the implementation of the workspace. Discussion: Interrelated barriers to collaboration were found at the professional, organisational and systemic levels. Some professionals could adapt the workspace to their existing tasks while others could not. Primary care providers need to strengthen their organisations while implementing the workspace. Conclusion: Concerted action at national and municipal level is needed to successfully implement digital tools.

  Purpose – New Public Management (NPM) has increased fragmentation in municipal health and social care organizations. In response, post-NPM reforms aim to enhance integration through service integration. Integration of municipal services is important for people with complex health and social challenges, such as concurrent substance abuse and mental health problems. This article explores the conditions for service integration in municipal health and social services by studying how public management values influence organizational and financial structures and professional practices. Design/methodology/approach – This is a case study with three Norwegian municipalities as case organizations. The study draws on observations of interprofessional and interagency meetings and in-depth interviews with professionals and managers. The empirical field is municipal services for people with concurrent substance abuse and mental health challenges. The data were analyzed both inductively and deductively. Findings – The study reveals that opportunities to assess, allocate and deliver integrated services were limited due to organizational and financial structures as the most important aim was to meet the financial goals. The authors also find that economic and frugal values in NPM doctrines impede service integration. Municipalities with integrative values in organizational and financial structures and in professional approaches have greater opportunities to succeed in integrating services. Originality/value – Applying a public management value perspective, this study finds that the values on which organizational and financial structures and professional practices are based are decisive in enabling and constraining service integration.

  Child poverty is an increasing challenge for Norwegian society and its municipalities. Policies to reduce it include housing as one particularly important health determinant. The capability approach allows us to focus on determinants for health. Methods were triangulated and literature and document studies, interviews and participating observations were part of the data-gathering process. Thematic analyses were used to analyse data. Children and adults view housing as important. Housing determines families’ possibilities of consuming other goods and choosing what functionings they will convert their resources into. Municipalities can change the course of development by engaging with the Norwegian Government Housing Bank and implementing instruments such as social housing, housing allowances and starter loans giving families the possibility to become homeowners. Housing determines intra- as well as inter-generational life chances. Housing is a corrosive functioning if children live in families that have low-quality housing or high housing costs. Housing is a positive functioning for children if families own their housing. Housing is defined as a market good and a welfare asset. Policies enable some families to buy their own housing, whereas the number of poor children still increases.

  This project studied how the Nordic countries apply policies to address the social determinants of health, and such a perspective demands awareness of the structural conditions for health. The project applied a qualitative approach using documents and interviews as data, and we explored whether the countries had comprehensive policies, if they included whole-society approaches, and if the policies were committing. Social inequalities may be termed a wicked problem, meaning that they are embedded in political conflict and thus it is difficult to find sustainable policy solutions. All countries have formulated political aims to reduce social inequalities, and such measures are mostly at the local level addressing lifestyle issues or social problems. The wider determinants of health are often not acknowledged, and policy problems are redefined from wicked to tame problems, which are less complex and conflicting.

  The gradient in health inequalities reflects a relationship between health and social circumstance demonstrating that health worsens as you move down the socioeconomic scale. Norway’s Public Health Act (PHA) specifically aims to tackle the gradient by addressing the social determinants of health. In this article, we draw on data from 2 studies that investigated how municipalities in Norway deal with these challenges. In doing so, we apply theoretical perspectives, as defined in the Gradient Evaluation Framework (GEF), to analyze the implementation of the PHA at the municipality level. The article aims to describe and analyze how local governments follow the requirements of the act. In doing so, we address the following research questions: Which policies are implemented at the local level to reduce social inequalities in health among families and children? How is intersectoral collaboration carried out, and who is taking part in the collaboration? The article draws on both quantitative survey data from questionnaires sent to all Norwegian municipalities and qualitative interview data in 6 municipalities. The findings show that there is raised awareness of the significance of social determinants among an increased number of municipalities, indicating that the PHA is being implemented according to its objectives.

  Aims: The gradient in health inequalities reflects a relationship between health and social circumstance, demonstrating that health worsens as you move down the socio-economic scale. For more than a decade, the Norwegian National government has developed policies to reduce social inequalities in health by levelling the social gradient. The adoption of the Public Health Act in 2012 was a further movement towards a comprehensive policy. The main aim of the act is to reduce social health inequalities by adopting a Health in All Policies approach. The municipalities are regarded key in the implementation of the act. The SODEMIFA project aimed to study the development of the new public health policy, with a particular emphasis on its implementation in municipalities. Methods: In the SODEMIFA project, a mixed-methods approach was applied, and the data consisted of surveys as well as qualitative interviews. The informants were policymakers at the national and local level. Results: Our findings indicate that the municipalities had a rather vague understanding of the concept of health inequalities, and even more so, the concept of the social gradient in health. The most common understanding was that policy to reduce social inequalities concerned disadvantaged groups. Accordingly, policies and measures would be directed at these groups, rather than addressing the social gradient. Conclusions: A movement towards an increased understanding and adoption of the new, comprehensive public health policy was observed. However, to continue this process, both local and national levels must stay committed to the principles of the act.

  Abstract Background: Norway is internationally known today for its political and socio-economic prioritization of equity. The 2012 Public Health Act (PHA) aimed to further equity in the domain of health by addressing the social gradient in health. The PHA’s main policy measures were (1) delegation to the municipal level of responsibility for identifying and targeting underserved groups and (2) the imposition on municipalities of a “Health in All Policies” (HiAP) approach where local policy-making generally is considered in light of public health impact. In addition, the act recommended municipalities employ a public health coordinator (PHC) and required a development of an overview of their citizens’ health to reveal underserved social segments. This study investigates the relationship between changes in municipal use of HiAP tools (PHC and health overviews) with regard to the PHA implementation and municipal prioritization of fair distribution of social and economic resources among social groups. Methods: Data from two surveys, conducted in 2011 and 2014, were merged with official register data. All Norwegian municipalities were included (N = 428). Descriptive statistics as well as bi- and multivariate logistic regression analyses were performed. Results: Thirty-eight percent of the municipalities reported they generally considered fair distribution among social groups in local policy-making, while 70% considered fair distribution in their local health promotion initiatives. Developing health overviews after the PHA’s implementation was positively associated with prioritizing fair distribution in political decision-making (odds ratio [OR] = 2.54; CI: 1.12-5.76), compared to municipalities that had not developed such overviews. However, the employment of PHCs after the implementation was negatively associated with prioritizing fair distribution in local health promotion initiatives (OR = 0.22; CI: 0.05-0.90), compared to municipalities without that position. Conclusion: Development of health overviews — as requested by the PHA — may contribute to prioritization of fair distribution among social groups with regard to the social determinants of health at the local level.   Abstract: Background: The Norwegian National government has developed public health policies that reflect health promotion principles, and these are particularly reflected in the recent Public Health Act (PHA). The counties (CMs) have been given a central role in the implementation of the PHA, and in this paper we explore how the CMs fill this role. Methods: Qualitative as well as quantitative data have been applied; a survey, a document study and personal interviews have been conducted. Results: The findings show that the CMs find it challenging to influence all sectors to change the focus from classical lifestyle issues to a focus on the social determinants of health. The Directorate of Health has the main responsibility for implementing the PHA, but the signals from the Directorate are not always consistent. The Directorate still launches campaigns and interventions to improve diets and stimulate physical activity, without launching them in the context of the PHA. Conclusion: The CMs regard the supporting role toward the municipalities as their highest priority. However, they find it hard to anchor and integrate the principles of the PHA. They explain this partly with the sectorised government organisation.

  Aims: One of the goals of the Norwegian Public Health Act is to reduce health inequities. The act mandates the implementation of policies and measures with municipalities and county municipalities to accomplish this goal. The article explores the prerequisites for municipal capacity to reduce health inequities and how the capacity is built and sustained. Methods: The paper is a literature study of articles and reports using data from two surveys on the implementation of public health policies sent to all Norwegian municipalities: the first, a few months before the implementation of the Public Health Act in 2012; the second in 2014. Results: Six dimensions are included in the capacity concept. Leadership and governance refers to the regulating tool of laws that frame the local implementation of public health policies. Municipalities implement inter-sectoral working groups and public health coordinators to coordinate their public health policies and measures. Financing of public health is fragmented. Possibilities for municipalities to enter into partnerships with county municipalities are not equally distributed. Owing to the organisational structures, municipalities largely define public health as health policy. Workforce and competence refers to the employment of public health coordinators, and knowledge development refers to the mandated production of health overviews in municipalities. Conclusions: The capacity to reduce health inequities varies among municipalities. However, if municipalities build on the prerequisites they control, establishing inter-sectoral working groups and employing public health coordinators in authoritative positions, national governance instruments and regional resources may sustain their capacity.

  Et hovedmål i samhandlingsreformen er å styrke det forebyggende helsearbeidet i kommunehelsetjenesten, blant annet fordi dagens helsetjeneste kritiseres for å prioritere ressurskrevende og spesialisert behandling fremfor å forebygge helseproblemer. I denne artikkelen spør vi om samhandlingsreformen har ført til økt fokus på forebyggende helsearbeid i kommunehelsetjenesten, og vi evaluerer om fallforebyggende tiltak, som et utvalg av kommuner utviklet som en oppfølging av samhandlingsreformen, har en sammenheng med hoftebruddsinsidens (antall nye tilfeller per år). Ved hjelp av en spørreundersøkelse finner vi at 119 kommuner har utviklet fallforebyggende tiltak. Ved å benytte data om hoftebruddsinsidens på kommunenivå fra Norsk pasientregister (NPR) finner vi at årlig hoftebruddsinsidens i kommuner som har gjennomført slike tiltak har gått ned med 0.09 hoftebrudd per 1 000 innbyggere relativt til kommunene som ikke har gjennomført slike tiltak, i perioden fra før til etter samhandlingsreformen. Sammenhengen er ikke statistisk signifikant. Når vi ser på hoftebruddsinsidens fordelt etter kjønn, finner vi at årlig hoftebruddsinsidens for kvinner har gått ned signifikant, mens det for menn er en positiv, men usikker, sammenheng.

  Worldwide, inequalities in health are increasing, even in well-developed welfare states such as Norway, which in 2012, saw a new public health act take effect that enshrined equity in health as national policy and devolved to municipalities’ responsibility to act on the social determinants of health. The act deems governance structures and “Health in All Policies” approaches as important steering mechanisms for local health promotion. The aim of this study is to investigate whether Norway’s municipalities address living conditions – economic circumstances, housing, employment and educational factors – in local health promotion, and what factors are associated with doing so. All Norway’s municipalities (n= 428) were included in this cross-sectional study, and both register and survey data were used and were subjected to descriptive and bi- and multivariate regression analyses. Eighty-two percent of the municipalities reported that they were capable of reducing inequalities in health. Forty percent of the municipalities defined living conditions as a main challenge in their local public health promotion, while 48% cited it as a main health promotion priority. Our study shows that defining living conditions as a main challenge is positively associated with size of municipality, and also its assessment of its own capability in reducing inequalities in health. The latter factor was also associated with actually prioritizing living conditions in health promotion, as was having established cross-sectorial working groups or inter-municipal collaboration related to local health promotion. This study underlines the importance of inter-sectoral collaboration to promote health and well-being.

  Aims: The public health coordinator (PHC) is a municipal-government position in Norway whose role is to organise and oversee municipal policies and functions to support national public health goals. This cross-sectional study investigates conditions associated with use of PHCs by Norwegian municipalities in the period immediately before the new Public Health Act came into effect in 2012, decentralising responsibility for citizen health to the municipal level. This study provides descriptive baseline data regarding Norwegian municipalities’ use of PHCs in this time – a marker for municipal engagement with inter-sectorial collaboration – before this policy was nationally mandated, and explores whether municipal characteristics such as structure, socio-economic status and extent of Health in All Policies (HiAP) implementation were associated factors. Methods: All Norway’s municipalities (N=428) were included. We combined Norwegian register data with survey data. Descriptive analyses and bi- and multivariate logistic regression analyses were performed. Results: A total of 76% of Norwegian municipalities employed a PHC in the period just before 2012. Of the PHCs employed, 22% were employed full time and 28% were located within the staff of the chief executive office. Our study indicates that partnership for health promotion with county councils (OR=7.78), development of a health overview (OR=3.53), collaboration with non-government sectors (OR=2.85) and low socio-economic status (OR=0.46) are significantly associated with Norwegian municipalities having a PHC. Conclusions: This study suggests that the municipality’s implementation of HiAP, as well as lower socio-economic indicators, is associated with the use of PHCs in Norway, but not factors related to municipal structure.

  The Norwegian National government has recently developed policies to reduce health inequalities; this should be achieved by a Health in all Policies approach (HIAP). Most policies are to be implemented at the local level and municipalities are responsible for the implementation. The aim of this paper is to answer this question: How can municipalities reduce social inequalities and among families with children through a mixed methods approach, using surveys as well as qualitative interviews in the collection of data? The main conclusion is that they are in different places when it comes to implementing public health and health promotion policies. The responsibility for public health and health promotion is to a large degree still left to the health sector, including those in positions as public health coordinators. To achieve HIAP, these positions should have an executive function in order to coordinate and influence local policies using a combination of universal and targeted measures. However, no extra funding has been allocated via the national budgets, and intensified efforts are still funded via projects. Still, we observe a change towards increased awareness of the HIAP approach to reduce social inequalities.

  Introduction: The two pillars of public health are health promotion and disease prevention. Based on a notion of governance in the state -local relation as changing from hierarchical via New Public Management (NPM) to New Public Governance (NPG), the governance of public health in Norway is contrasted to governance of public health in the other Nordic states: Denmark, Finland and Sweden. Aim: The article aims to present and discuss the governance of public health as it is played out in the state-local relationship. Methods: The method is to study central state documents in the four countries, as well as articles, research reports and papers on public health. Results: The article shows that the governance modes (hierarchy, NPM and NPG) exist in parallel, but that their mechanisms actually vary in use. Legal, economic and informational mechanisms are, to a varying degree, in use. Conclusions: In Finnish and Swedish public health policies, health promotion is at the forefront; while Danish and Norwegian public health policies spur the local governments to carry out interventions to prevent disease and hospital admissions.

  Abstract til konferansen «Velferdsstat og ulikhet i helse i økonomiske nedgangstider», sesjon: «Kommunenes arbeid med å redusere sosial ulikhet i helse» Background and research question In 2012 the Norwegian government adopted a public health act. The basic principles is five, among them it relies heavily on redistributive policies and Health in All Policies that, among others, mandates municipalities to make health overviews of positive and negative factors that creates health. Our main research questions are; how are municipalities handling this challenge and what kind of policies are implemented locally? Secondly, we look into the coordination of local policies and actors. Theoretical framework The theoretical framework is multilevel governance. The perspective argues that the cooperative turn has taken place and that municipalities can develop policies on equal standing with the state level. As such they can decide on their own policies because they possesses the relevant knowledge, and that also other relevant actors can be involved at municipal level. Further, there is mutual dependency among the national, regional and municipal level and arenas have to be established for coordination and discussion of knowledge. Methodological approach Analyses will rely on triangulation of methods. Firstly we use quantitative data from two surveys carried out in Norwegian municipalities in 2011 and 2014. Secondly, these analyses are used as a basis for analyzing interviews carried out with key actors in political and administrative leaders, as well as leaders at agency level. Third, we use literature studies and document studies of local plans. Results We observe that the cooperative turn has not taken place to it` s full extent. Municipalities most commonly report to develop policies to change individual health behaviour nevertheless, they believe to be able to reduce inequities in health. Interviews show that they include redistributive policies in their plans and measures. About 40 per cent of municipalities have made health overviews, not many have used it to formulate and prioritise policies. We see that municipalities who include health overviews as a basis for their planning more often have inter-sectoral work groups than others. Municipalities as well employ public health coordinators to support coordination between the sectors of health and planning. Implication for research We observe a development between the years and more municipalities have made health overviews in 2014 than in 2011, nevertheless many await the national policies before they start their own work. This means they still lack the necessary knowledge and expertise. To ensure this research must be carried out to evaluate policies and measures implemented at the local level.   Velferdspiloten er initiert og drevet av Fylkesmannen i Oslo og Viken med deltakere fra NAV, Bufetat Øst, Husbanken Øst og Viken Fylkeskommune. Kommunene som deltar er Fredrikstad, Indre Østfold, Sarpsborg og Halden. Velferdspiloten har definert fem mål de arbeider for å nå; å utvikle en ny modell for samordning, identifisere og endre uhensiktsmessige regler og arbeidsmetoder og løfte slike til nasjonalt nivå, mer effektiv ressursbruk i kommunene og hos statlige og regionale aktører, økt innovasjon i kommunene og å dele erfaringer. Utgangspunktet for denne andre rapporten om Velferdspiloten er tre forskningsspørsmål som ble formulert som en følge av erfaringene fra den første rapporten og beskriver både den horisontale og den vertikale samordningen mellom de regionale og kommunale aktørene i arbeidet mot barnefattigdom. Rapporten er basert på en kombinasjon av intervjuer, observasjon av møter, samt gjennomgang av møtereferater fra møter i arbeidsgruppen og fra møter med kommunene. Rapporten viser at Velferdspiloten har bidratt til at det er blitt avdekket retningslinjer og arbeidsmetoder som kan være uhensiktsmessige. I kommunene som Velferdspiloten har samarbeidet med ser vi at den også har bidratt til både innovasjon, og mer effektiv utnyttelse av ressursene. Særlig er dette synlig i det økte handlingsrommet som den vertikale samordningen mellom kommunene og Velferdspilotens har bidratt til å skape. Det har også vært en styrke for innovasjonsarbeidet at både det horisontale samarbeidet på regionalt nivå, og det vertikale samarbeidet mellom regionalt nivå og kommunene var åpent for mange mulige utviklingsveier. Dette gjør det, på begge nivåer og i relasjonen dem imellom, mulig å styrke og utvikle strategier som er basert på kunnskap og innsikt i de konkrete organisatoriske faktorene i kommunene og hvor det vertikale samarbeidet kan bidra med midler, kompetanse og verktøy.

  Denne første del av følgeevalueringen av Kompetanseløft 2020 (K2020) viser at svært mange kommuner søker midler over K2020, noe vi vil anta viser at K2020 svarer på reelle behov i kommunene. Så langt kan det virke som det kommunale kompetanse- og innovasjonstilskuddet ikke ses på som et lavterskeltilbud ved at kommunene utelukkende prioriterer kursopplæring for ansatte. Kursopplæring er den hyppigste formen for opplæringstiltak, men bare seks kommuner benytter seg kun av dette tilbudet. Høyere utdanning i form av videre- og masterutdanning er det vanligste etter kurs, noe som kan vitne om at kommunene (og ansatte) ønsker forskningsbasert kunnskap og dybdekompetanse i tjenesten. De aller fleste kommuner jobber planmessig ved at de skaffer seg oversikt over behov og kompetanse, og kommunene lager kompetanseplaner. Det ser ut til å være vanligere at store kommuner lager en egen kompetanseplan for helse- og omsorgstjenesten enn at små kommuner gjør dette, men mange kommuner innarbeider kompetanseplan for helse- og omsorgstjenesten i annet planarbeid. I alt ca. 80 prosent av de 215 som svarte på spørreskjemaet kartlegger eget behov for kompetanse, noe som er en forutsetning for både tjeneste- og fagutvikling for å møte både erkjente, nåværende behov i kommunen og for å legge til rette for videre utvikling for å møte forventede, framtidige behov. Det er foreløpig for tidlig å si om det er systematiske forskjeller i bruken av de ulike typer midler og om det er systematiske forskjeller mellom kommuner.

  Folkehelse og forebygging er brede samfunnsoppgaver som Folkehelseloven har lagt til kommuner og fylkeskommuner. Prosjektet presenterer og analyserer spørsmål om hvem som er målgrupper for dette arbeidet, hva som er strategiene for å nå målgruppene, om kommuner iverksetter kunnskapsbaserte tiltak og om de mener at de har effekt av tiltakene og selv kan styre hvilke tiltak de skal sette i verk.

  Last ned gratis Kompetanseløftet 2015 (K2015) er regjeringens kompetanse og rekrutteringsplan for de kommunale helse- og omsorgstjenestene. Evalueringen er et samarbeid mellom NIBR og NIFU. Den studerer kommunenes bruk, og deres oppfatning av nytteverdien av K2015. I dette notatet er det gjort analyser av konteksten for kommunenes rekrutteringsarbeid innenfor denne sektoren – studieplasser, antall studenter og antallet uteksaminerte på vgs og høgskolenivå, samt andelen som tar ulike typer videre- og etterutdanning. Det har ikke vært store endringer i dette de siste årene. Kommunene søker midler fra K2015 og de har en positiv oppfatning av nytten av kompetanse- og rekrutteringsplanen.   Prosjektet er en litteraturstudie og tar for seg artikler som sier noe om sammenhengen mellom bolig/boområde og helse. Litteraturstudien finner at det er en sammenheng mellom bolig/boområde og helse, og at det å bo i en dårlig bolig påvirker helsen negativt, både den fysiske og den mentale helsen. Denne sammenhengen kan overstyres av variablene utdanning og inntekt. Det tyder på at det er sosiale forskjeller i fordeling av bolig som er de bakenforliggende årsakene.   En nasjonal oversikt over status i kommuner to år etter reformoppstart med et spesielt blikk på innvandrerrike kommuner. Dette prosjektet gjør opp status for kommunenes helsefremmende og sykdomsforebyggende arbeid to år etter reformoppstart. Prosjektet fokuseres på tre dimensjoner ved arbeidet. For de første, hva slags forståelse av folkehelse som har nedfelt seg blant kommunenes ansatte? For det andre hvordan folkehelsearbeidet organiseres, koordineres og hvilke samarbeidsrelasjoner som utvikles. For det tredje hvilken type tiltak som implementeres. Noen sykdommer er spesielt utbredt blant deler av innvandrerbefolkningen og prosjektet vil ha et spesielt fokus på innvandrerrike kommuner.   Dette er første delrapport i følgeevalueringen av etter- og videreutdanningstilbudet Helse og omsorg i plan. Rapporten viser hvordan KS/SEVS og undervisningsinstitusjonene har løst oppdraget fra Helse- og omsorgsdepartementet/ Helsedirektoratet, studentenes forventninger til kurset og hvordan de vurderer kurset i etterkant. På bakgrunn av disse resultatene gir rapporten anbefalinger for videre utvikling av etter- og videreutdanningstilbudet.

  I studien rettes oppmerksomheten mot de statlige pedagogiske styringsvirkemidlene overfor kommunene og spørsmålet om hvordan hensynet til lokal variasjon er ivaretatt i disse. Veiledere er valgt ut fra områdene helse, skole og plan, og de drøftes med utgangspunkt både statlige myndighetsorganer som har utferdiget veilederne og i kommunenes opplevelse av disse.Vi finner store forskjeller både i hvordan veilederne er utformet og i hvordan de tilrettelegger for kommunal variasjon og kommuniserer sin status.

  Rapporten presenterer en kartlegging av folkehelsearbeid regionalt og lokalt. Det fokuseres på regionale og lokale ressurser, hvordan nivåene har organisert arbeidet og hvordan det koordineres. Undersøkelsen er en baseline med fokus på situasjonen for folkehelsearbeid før Folkehelseloven av 2011 ble satt i verk.

  Kommunene får større ansvar for velferdsstatens tjenester. I sin styring av kommunene benytter staten blant annet juridiske og pedagogiske virkemidler. Rapporten presenterer observasjoner av hvordan kommunale aktører oppfatter at slike virkemidler styrer den kommunale helse- og omsorgssektoren.

  «Flink med folk i første rekke» (FFFR) har vært et prosjekt i Kompetanseløftet som er en del av Omsorgsplan 2015, regjeringens rekrutterings- og kompetanseutviklingsplan. FFFRs hovedmålsetting har vært å medvirke til at kommunene utvikle ledelseskompetanse innenfor pleie og omsorg samt psykisk helsearbeid. Den utviklede kompetansen skulle være merkbar for brukere av kommunenes tjenester. Kompetansen skulle utvikles innenfor tverrfaglig arbeid, brukermedvirkning og systematisering av bruk av individuelle planer, etisk refleksjon, helhetlig og utfordrende ledelse, kompetente medarbeidere, samt kompetanseplanlegging og utvikling. Kommunene har gjennomført prosjekter lokalt der de satset på ett eller flere av disse temaene. Hovedobservasjonen i evalueringen er at problemstillingene for FFFR var aktuelle for kommunene og at de har arbeidet godt med prosjektene de satte i gang. Det er spesielt mange kommuner som har arbeidet med å lage en kompetanseplan og brukt KS' verktøy for slike planer. Helsedirektoratet har vært prosjekteier, mens KS (Kommunenes interesse- og arbeidsgiverorganisasjon) har gjennomført prosjektet og iverksatt det i sitt samarbeid med kommunene.

  I 2007 fikk NIBR og NIFU i oppdrag å evaluere Kompetanseløftet 2015. I denne rapporten sees det nærmere på hvordan Kompetanseløftet er blitt iverksatt, og da særlig i kommunene. I tillegg innholder rapporten oppdaterte data fra NIFUs tidligere underveisrapporter om oppnåelse av måltallene. Hovedinntrykket er at kommunenes arbeid ikke har endret seg vesentlig siden Opptrappingsplanen for psykisk helse ble avsluttet i 2008. Rapporten ser på organiseringen av det psykiske helsearbeidet blant voksne og blant barn/unge.

  Våre respondenter i fylkesmannsembetene var i 2009 gjennomgående godt fornøyd med den løpende kontakten med kontaktpersonene i Helsedirektoratet. Det ser altså ut til at styringen og samordningen fungerer godt på dette nivået.Samarbeidet mellom fylkesmannsembetene og kommunene ser også ut til å fungere godt. Det største problemet til kommunene er av økonomisk art. Tilskuddenes beskjedne størrelse og problemer med å få dekket vikarutgifter fremstår som Kompetanseløftets akkileshæl.

  Denne rapporten er ei oppsummering av dei tre folkehelsetiltaka Partnarskap for folkehelse, Helse i plan og Kommunehelseprofilar. Målet med rapporten er å gje ei kortfatta oversikt over kva dei enkelte tiltaka går ut på, korleis dei er organiserte og kva erfaringar fylka og kommunar har gjort seg i arbeidet med desse tiltaka. Rapporten summerer også opp forhold som er viktige for at ordningane skal fungere best mogleg. I tillegg til dei tre nemnde tiltaka, er Tilskotsordninga til lavterskel fysisk aktivitet evaluert. Resultata frå evaluering av ordninga er ikkje inkludert i den oppsummerande rapporten, men finst i rapporten Fra sofaen til sykkelsetet. Evaluering av Tilskotsordninga til lavterskel fysisk aktivitet (Bergem, Ouff, & Aarflot, 2008). Rapporten går ikkje inn på metodisk og teoretisk forankring av evalueringsarbeidet. Den går heller ikkje i djupna på viktige analysar som dannar grunnlag for forskarane sine avsluttande kommentarar.
